During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 12/10/2025 and completed on 12/12/2025 in response to several related complaints received by the licensing office on 12/04/2025 and 12/10/2025 relating to allegations of ratio and cleanliness. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/23/2025. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center shall be maintained in a clean and safe condition.

During the inspection, rodent droppings were observed in the kitchen area.
Plan of Correction: A workorder was placed on 12/12/2025 for the exterminator to come out to the center. Work order was completed on 12/15/2025 at 1:40 pm by Terminix.

The required ratio for children under the age of 16 months is one staff per four children.

On multiple occasions between 12/1/2025 and 12/5/2025, the Toddler room had 10 children (the youngest child being 14 months) with two staff members.
Plan of Correction: All students transitioning to a new classroom will have a transition packet on file with staff signatures, management signature, and family signature to include: Transition Letter, Age-Up Letter, and Developmental Assessment.

The required ratio for children 16 months to 24 months is one staff per five children.

On multiple occasions between 12/1/2025 and 12/5/2025, the two year old classroom had more than 10 children (the youngest child being 18 months) with two staff members.
Plan of Correction: All students transitioning to a new classroom will have a transition packet on file with staff signatures, management signature, and family signature to include: Transition Letter, Age-Up Letter, and Developmental Assessment.

On multiple occasions between 12/1/2025 and 12/5/2025, the preschool classroom had more than 8 children (the youngest child being 2 years old) with one staff member during the morning drop-off period.
