During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 03/24/2026, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as a result of the last inspection related to supervision. The on-site inspection began at 10:00 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 60children present and 13 staff. This inspection included: document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Violations issued were not to related to the areas covered by the IPOC.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/31/2026. A POC submitted after this date will not appear on the public website.

Hazardous substances such as cleaning materials shall be kept in a locked place, using a safe locking method that prevents access by children.

Cleaning supplies were found in an unlocked low cabinet, that the children had access to.
Plan of Correction: Facilities was on site on 3/24/2026 to repair the finger gaurd on the cabinet.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

A power strip with three vacant outlets was observed in the toddler class, without outlet covers in them.
Plan of Correction: All outlets, to include power strips, have been covered with outlet covers. Additional outlet covers have been purchased to store on site.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

The foot operated trash can, was placed on an elevated surface, where the foot pedal was not available to the staff member in the toddler room.
Plan of Correction: Trash can with foot pedal has been placed on the floor next to the changing table, where teachers have access to the foot pedal at every single diaper change.
