Inspector Notes

An unannounced monitoring inspection was conducted on 09/22/2022 from 10:15 a.m. to 10:50 a.m. There were 5 children present, ranging in ages from 22 months to 4 years old with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 1 staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the provider failed to keep a written record of children in attendance each day.
Evidence:
1. An attendance sheet was not found during the inspection.
2. The provider confirmed that a written record of the children in attendance was not kept the day of the inspection.
Plan of Correction: Provider will make sure attendance sheet is posted daily.
