Inspector Notes

An unannounced monitoring inspection was conducted on 11/14/2023 from 10:20 a.m to 11:10 a.m. There were five children present, ranging in ages from 10 months to three years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, one household member?s record and one staff record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the provider failed to obtain for each caregiver and adult household member a current tuberculosis screening every two years.
1. The date of the provider?s last tuberculosis screening was10/25/2021. The date the household member #1?s last tuberculosis screening was10/25/2021.
2. The provider confirmed that the tuberculosis screenings had not been repeated every two years.
