Inspector Notes
A focused monitoring inspection was initiated and concluded on 2/11/2022 to review compliance during the Provisional Licensure period. There were 21 children present, ranging in ages from 5 months to 3 years old, with 3 staff supervising. Children (other than infants) were napping at the time of the inspection. The inspector reviewed compliance in the areas of physical plant, staffing and supervision, General Procedures.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Standard 8VAC20-820-120-E-2
Based on observation and interview, the licensee did not visibly post the most recent findings (violation notice) of the most recent licensing inspection of the program.
Evidence:
The last inspection Summary was posted on a parent information board located just inside the entrance used by parents to enter the center. The violation notice was behind the summary page and was not visible. There was a clear plexiglass cover over the board enclosing the posted items. The administrator removed the document from the board and showed the inspector that the violation notice was behind the Summary page stating that it was inadvertently posted backwards.

The last inspection Summary was posted on a parent information board located just inside the entrance used by parents to enter the center. The violation notice was behind the summary page and was not visible. There was a clear plexiglass cover over the board enclosing the posted items. The administrator removed the document from the board and showed the inspector that the violation notice was behind the Summary page stating that it was inadvertently posted backwards.
Plan of Correction: The administrator rearranged the posted document on 2/14/2022 with the violation notice on top so that it is clearly visible.

Based on observation and interview, the provided notice of the commissioner's intent to take actions enumerated in ? 63.2-1709.2 of the Code of Virginia was not posted in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.

The inspector did not observe the Notice of Intent posted in the facility during the inspection on 2/11/2022.
The administrator confirmed that the Notice of Intent was not posted and stated that she was not aware that she was required to post it.
