Inspector Notes
A monitoring inspection was initiated and concluded on 5/8/2024. There were 20 children, ages 10 months to 4 years of age present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed washing hands and eating lunch which consisted of macaroni and cheese, green beans, mandarin oranges, and milk. Other activities observed were story time, planting seeds in cups of soil, music and movement, and rest time. Infants were observed being held, rocked and bottle fed, having tummy time, crawling on the carpet and resting in cribs. A total of 5 child records and five staff records were reviewed. The inspection started at 11:10 am and concluded at 1:55 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Five children's records were reviewed.
The record for child #1 contained an immunization record which was not signed (or stamped) by a physician, his designee, or an official of a local health department.
The record for child #2 contained the results of a physical examination which were not signed (or stamped) by a physician, his designee, or an official of a local health department.
The parent s of child #1 and child #2 will be asked to provide a signed copy of their immunizations and physical results for their child's file within 2 weeks.
Evidence:
A blue two-seat plastic rocker was located on the playground without resilient surfacing in the fall zone / under and around it.
The rocker seats were approximately 12 inches from ground level.
