Inspector Notes
An unannounced, on-site monitoring inspection was initiated on November 4, 2024 and completed on November 6, 2024. The on-site inspection began at 10:00 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were fifty-one children present and twelve staff. The inspector reviewed five children?s records and four staff records on-site and electronically. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 14, 2024. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #5, who had been enrolled for five weeks.

(Repeat violation) Immunization records and physical examinations are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for Child #1 was not signed. The physical examination for Child #2 was not signed.

On play areas, ground supports shall be covered with materials that protect children from injury. The ground supports under the school bus and 4 seater spring rocker were exposed and not covered by mulch.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for Child #1, and Child #4, each enrolled for over a year, were missing information for two emergency contacts.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff records are to have documentation of an emergency contact. Staff #1, who has been actively working at the center for over a week, did not have any completed reference checks or emergency contact on file.
