A subsidy health and safety inspection (SHSI) completed on 5/16/2025. The inspection started at 9:20 am and concluded at 10:00 am. There were (2) children present and (1) caregiver, the point total was (4). The inspector reviewed (3) children?s records and (4) caregiver /household member records on-site.
