Inspector Notes

A monitoring inspection was initiated and concluded on 04/26/2022. There were 93 children present, ranging in ages from 6 months to 5 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation of the playground it was determined that not all areas were in clean, safe, and operable condition. Evidence: 1. The drain box on the right side of the playground has sharp edges protruding from the ground that could snag skin or cause a tripping hazard. 2. The lumber barrier around the playground structure has a nail protruding (4 threads) that could snag skin or cause a tripping hazard. 3. There is a large birds nest attached to a wind chime outside of the door to the playground. Bird excrement was observed on the ground and wall below the nest.
Plan of Correction: Landscaping was contacted to fix water drain, repair lumber barrier, and remove broken chime with bird's nest and also pressure wash wall and sidewalk.

Based on observation and interviews with administration it was determined that the facility failed to maintain a ratio of one staff to 5 children ages 16 months - 2 years. Evidence: Two staff members were observed caring for 15 children in the toddler room with ages ranging from 16 months - 2 years.
Plan of Correction: More staff has been hired to help supplement the facility being short staffed.
Standard 8VAC20-780-80-A
Based on interviews and review of attendance records it was determined that not all attendance records documented the arrival of children as it occurred. Evidence: There were 8 children observed in the older infant room with only 4 children checked in on the daily attendance record. The time of arrival for child A, B, C, and D were not documented on the attendance record.
Plan of Correction: Staff are being retrained on marking children in on both the attendance sheet as well as on the tablet when they arrive and leave during the day.
