An investigation was initiated from 12:00 pm - 2:45 pm to investigate a complaint regarding care, protection, and guidance. Interviews were conducted, photos, documentation, and records were reviewed. The complaint regarding care, protection, and guidance, is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interviews it was determined that staff did not always ensure the care, protection, and guidance of children under their supervision. Evidence: On 01/30/2023 staff could not explain how child A (Age 4) woke up from nap with a round, blistered mark on their right nostril. Staff confirm that child A did not have a mark on their face before nap. Staff were not able to provide an explanation as to how Child A sustained the injury to their nose. Child A did not articulate how the mark came to appear on their nose.
