An unannounced complaint inspection was conducted on 12/13/23 to investigate an allegation received by the department related to special care provisions and emergencies. Based on evidence gathered through interviews, observations and documentation review, the complaint was determined to be Not Valid. There was, however, a violation cited as a result of the inspection which is documented on the violation notice issued to the facility. An exit interview was conducted on 12/13/2023. (Repeat Violation) Based on review of two staff records, it was determined that not all staff had an original central registry report within 30 days of employment, and no documented follow up was conducted. Evidence: the record for staff #2 (date of hire: 11/7/2022) did not contain documentation of a central registry search report or follow up.
