During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 1/8/2025, as a part of the provisional licensure period. The on-site inspection began at 9:30am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 83 children present and 14 staff. The inspector reviewed 7 children?s records and 7 staff records on-site.

This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

The written allergy list throughout the building did not include Child #1, with a diagnosed food allergy.
Plan of Correction: The missing child's name has been added to the list and all lists have updated and distributed to the classrooms.
Standard 8VAC20-780-520-C
When diaper ointment or cream is used these products do not need to be kept locked but shall be inaccessible to children.

There were several tubes of diaper creams in a bin and topical skin lotion and handwash on a low bathroom sink in the Panda?s classroom, accessible to children.
Plan of Correction: The products were immediately removed and moved to a location that will be inaccessible to the children.
Standard 8VAC20-780-550-E
Shelter in place procedures shall be practiced a minimum of twice per year.

The center only practiced one shelter-in-place drill from January 2024 to January 2025.
Plan of Correction: An extra lockdown drill was conducted instead of the shelter-in-place. Center now knows that 2 shelter-in-place drills need to be conducted annually.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #2, enrolled for 2 days, was missing work phone number and place of employment of each parent.
The record for Child #3, enrolled for 2 months, was missing the name, home address, and home phone number of one parent.
The record for Child #4, enrolled for 3 years and 2 months was missing work phone number and place of employment of each parent.
The record for Child #5, enrolled for 1 year and 2 months, was missing the name, home address, and home phone number of one parent.
Plan of Correction: The center will request information from parents to add to the enrollment form.
