During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on January 28, 2025. The on-site inspection began at 8:45 a.m. and ended at 10:15 a.m. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 20. The inspector reviewed 5 children?s records and 2 caregiver records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 4, 2025. A POC submitted after this date will not appear on the public website.

The findings from the most recent inspection, dated 8/5/2024, were not posted in a conspicuous place on the premises.
There were no physical exam records available for Child #1, who had been in care for over 9 months.
Child #1, 16 months old, Child #2, 15 months old, and Child #3, 13 months old, were observed in a high chair and the safety strap was not used.
Caregiver #2 was observed changing a diaper and did not wash their hands afterwards.
The parents of Child #3 and Child #4 were not provided in writing the requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease. Children had been in care for over one year.
Child #1, enrolled for 9 months, did not have a parent acknowledgment.
Records for Child #2, Child #3, and Child #4 did not have a parent?s signature for an annual review of the emergency contact information. Child #2 had been in care for over a year, and their annual review was overdue by 16 days. Child #3 had been in care for over one year and 5 months, and their annual review was overdue by 5 months. Child #4 had been in care for two years and 10 months, and their annual review was overdue by one year and 10 months.
The records for Child #1, Child #2, and Child #3 were missing documentation of the method of verification of age and identity.
The last date of an emergency evacuation procedure was on 10/4/2024 and overdue by two months.
The last date of a shelter-in-place procedure was on 1/5/2024 and did not have a second drill completed.
Child #1, 15 months old, last documented immunization was over 9 months ago. Child #2, 16 months old, last documented immunization was over 9 months ago.
