During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated on and completed on 3/27/2026. The on-site inspection began at 9:30am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were two children present and two caregivers; the point total was six. The inspector reviewed three children?s records and two caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
7The findings from the most recent inspection dated 8/4/2025 was not posted in a conspicuous place on the premises.
There were no physical exam records available for Child #2 who has been in care for 11 months.
Caregiver #2 who has been working with children for two weeks had documentation of only one reference.
Caregiver #2 who has been caring for children for two weeks did not have age verification documented.
Caregiver #2 has been caring for children for two weeks does not have documentation of completing orientation.
The last date of an emergency evacuation procedure was in December 2025 and overdue by three months.
