Inspector Notes

An unannounced renewal inspection was conducted July 1, 2024, between the hours of 9:40am - 11:25am. There were 6 children (14 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 3 children?s records, emergency supplies and policies were inspected. Children were observed playing with caregivers and playing outside. There was an assortment of age-appropriate materials for children. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website. Standard 22.1-289.035-B-2
Based on record review and interview, the family day home did not ensure that all caregivers have the result of a fingerprint based national criminal history search prior to the first date of employment.
Evidence:
1. The record for caregiver #3 (date of hire: 06/17/2024) had documentation of a fingerprint based national criminal history search dated 06/20/2024.
2. Caregiver #1 acknowledged that Caregiver #3 did not have documentation of a fingerprint based national criminal history search prior to the first date of employment.
Plan of Correction: Caregiver 3 was hired directly from FCPS on 6/15 and began their training on 6/17. Caregiver 3 obtained their background checks on their first day of training on 6/17 and continued training through 6/21 during which was not left alone with any children.
Results were obtained on 6/21on their 4th day of training. LSP LLC will ensure that prior to training all employees will have completely obtained results.
Standard 8VAC20-800-120-B
Based on observation and interview, caregiver records did not contain all required information.
Evidence:
1. Caregiver # 3's (date of hire: 06/17/2024) record did not contain documentation of verification of age.
2. Caregiver # 3's record did not contain documentation that two or more references as to character and reputation as well as competency were checked prior to employment.
3. Caregiver #1 acknowledged that Caregiver #3 was missing documentation of age verification and documentation of two references.
Plan of Correction: Caregiver 3 was hired directly from FCPS for work over the summer. Caregiver 1 completed several verbal references 6/10-6/14 with prior supervisors at the school prior to employment as well as verified caregiver 3s driver license on 6/15 prior to first day of training to assist in scheduling background checks. Caregiver 1 will ensure all employees have copies of age verification and references on file going forward.
Standard 8VAC20-800-170-B
Based on record review and interview the family day home did not obtain from each caregiver a tuberculosis screening dated within the last 30 days prior to date of hire.
Evidence:
1. Caregiver #3 (date of hire: 06/17/2024) had documentation of a negative tuberculosis (TB) test dated 08/14/2023.
2. Caregiver #1 acknowledged that Caregiver #3's TB test was older than 30 days prior to date of hire.
Plan of Correction: Caregiver 3 worked for FCPS prior to employment with LSP LLC and had a TB test from 8/14/2023. Caregiver 1 did not know it had to be withing 30 days prior to hire. but believed it had to be every 2 yrs as it is once they have one completed. Licensing reoriented caregiver 1 on the regulations so that all caregivers will have TB screenings completed within 30 days prior to hire.
Standard 8VAC20-800-200-A
Based on record review and interview, the provider did not orient the assistant provider by the end of their first week of assuming job responsibilities.
Evidence:
1. Caregiver #3's (date of hire: 06/17/2024) record did contain documentation of orientation.
2. Caregiver #1 acknowledged that Caregiver #3 did not receive orientation training.
Plan of Correction: Caregiver 1 actually did complete caregiver 3's orientation between the dates 6/17-6/21. Caregiver 1 forgot to have caregiver 3 sign the document stating that orientation had taken place. Caregiver 3 signed the document on 7/1/24 once it was brought to caregiver 1s attention upon inspection.
Standard 8VAC20-800-780-B
REPEAT VIOLATION:

Based on record review, all childrens records did not contain parents signed acknowledgement that the emergency contact information was reviewed annually.
Evidence:
1. Child #2's (date of enrollment: 10/23/2021) last annual emergency information contact review was dated 8/11/2022.
Plan of Correction: LSP, LLC will have all parents complete annual reviews at the same time to ensure no files are missed. All annual reviews will be completed in July.
Standard 8VAC20-800-830-A
Based on record review and interview, emergency evacuation procedures were not practiced monthly with all caregivers and children in care.
Evidence:
1. There was no documentation of emergency evacuation procedures being practiced available for review.
2. Caregiver #1 acknowledged that emergency evacuation procedures had not been practiced monthly.
Plan of Correction: Emergency Drills document was printed on 7/1 after licensing visit and a drill was completed on 7/2.
