Inspector Notes
An unannounced monitoring inspection was conducted today between 1:00pm and 2:00pm. There were 8 children (20 points) present with the Provider and 1 assistant from 1:00-2:00pm within the supervision guidelines. From 2:00-3:00pm, there were 8 children (20 points) with only the Provider. The children were observed taking a nap as it was their rest period. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

1Evidence:
1. From 2:00pm-3:00pm, the Provider stated that their assistant left from the family day home, leaving the Provider over the points allotted per caregiver. The Provider had 8 children (20 points) by himself during this time period.
