Inspector Notes

An unannounced renewal inspection was conducted today between 10:10am-10:55am. There were 7 children present with the Provider and 2 assistants for a total of 20 points within the supervision guidelines. The children were observed playing outside on the climbing structure and riding tricycles. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and caregiver records, evacuation drill log, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Based on a review of caregiver records, all Assistants' records did not contain the required information.

Evidence:
1. Caregiver # 1 (doh: 1/3/2024) did not have documentation on file of 2 out of 2 required references prior to employment at the FDH.
Plan of Correction: I will get the references and make sure I conduct the reference checks moving forward when hiring new assistants.
