Inspector Notes

A monitoring inspection was initiated on May 3, 2022 and concluded on May 3, 2022. There were 11 children present, ranging in ages from kindergarten to 12 years of age with one staff supervising. The inspector reviewed administration, personnel, household members, caregiver training, physical equipment and environment, care of children, preventing the spread of disease, medication administration, emergencies, nutrition, transportation, background checks, provider must be MAT certified to administer prescription medication, and child abuse and neglect. A total of two child records, one caregiver record and one householder member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
1. The record for caregiver #1 (date of last TB screening 11/22/19) contained updated TB screening results of 03/24/22.
2. The record for Household Member #1 (date of last TB screening 12/05/19) contained updated TB screening results of 03/24/2022.
