Inspector Notes

A monitoring inspection was conducted July 10, 2023 from approximately 9:50 AM to 11:20 AM. There were six school age children present with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition.. A total of five child records, one staff and one household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

Based on documentation review and interview, the caregiver did not ensure the emergency plan was documented as being reviewed and updated at least annually.

Evidence:
The plan was documented as last being reviewed on 03/20/2022. The caregiver stated the plan has been reviewed but the review was not documented.
Plan of Correction: Provider reviewed and signed for the annual review during the renewal inspection.
