Inspector Notes

An unannounced renewal inspection was conducted on 4/26/22 from 9:30am - 11:15am. During the inspection there were 38 children ages five months old through five years old in care with seven staff. During the tour children were observed participating in various activities in the classrooms. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were also reviewed during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of the medication being stored at the facility, it was determined that the facility did not ensure that in order to administer prescription medication, the center has obtained written authorization from a parent or guardian, and that any medication that was dispensed from a pharmacy is maintained in the original, labeled container.

Evidence:
1. The written authorization for the medication (Albuterol inhaler) for child #6 was not signed by the parent.
2. Staff #2 (Program Director) confirmed that there was medication at the facility that did not have written authorization from the parent in order for it to be administered.
Plan of Correction: The facility responded: The parents of child #6 will be asked to sign the medication authorization form or the medication will be sent home. Going forward all medication will not be accepted unless it has a medication authorization that is signed by the parent.
