Inspector Notes

An unannounced renewal inspection was conducted on 9/6/22 from 11:30am - 1:15pm. During the inspection there were 47 children ages two months old through four years old in care with 12 staff. Children were observed participating in various activities in the classrooms, eating lunch, and during rest period. Records were reviewed for six children and five staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.

Based on a review of documentation and interview, it was determined that the facility did not ensure that an annual approval from the health department shall be provided.

Evidence:
1. The most recent annual health inspection report inspection available for viewing during the inspection was dated 8/23/21.
2. Staff #6 (Program Director) confirmed that the annual inspection from the health department had not been completed.
Plan of Correction: The facility responded: The local health department will be called to schedule the inspection.
