Inspector Notes

A monitoring inspection was conducted on 08/29/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed records and (#) staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
Two infants were observed sleeping in cribs that had sheets that did not have snug to the mattress, posing a suffocation hazard.
Plan of Correction: Management will ensure all crib sheets fit snug to the mattresses.
