Inspector Notes

An unannounced, on-site monitoring inspection was completed on 08/06/2024 as a part of licensure period. The on-site inspection began at 9:45am and ended at 11:30a. The inspector reviewed compliance in the areas listed above. There were 42 children present and 11 staff. The inspector reviewed 4 children?s records and 4 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation the center did not ensure all areas and equipment of the center were maintained in safe condition.
Evidence:
In the preschool classroom there was a pair of adult scissors that were pointed on a table in reach of children.
Plan of Correction: The adult scissors were removed from the classroom during the inspection.
Standard 8VAC20-780-440-H
Based on observation the center did not ensure all cribs met the requirement, that there
shall be no more than one inch between the mattress and the crib.

Evidence:
There were 5 babies observed in their cribs. In the corner of each crib, there was more than one inch of space between the mattress and the crib.
Plan of Correction: The director will remind staff to ensure the mattress is smooth out and flat in the mattress sheet to prevent the ends from curling in. Alternative sheets will also be explored.
