During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on March 31, 2025. The inspection started at 9:35am and ended at 10:15am. There were no children present at the time of the inspection. The inspector reviewed three children's records and three caregiver/household members records on-site.

Mattresses when used shall be covered with a waterproof material that can be cleaned and sanitized. There were two small cloth mattresses/long cushions for the children to rest on that did not have a waterproof cover.
Plan of Correction: I plan to buy waterproof kid mats by 4/14/25.
Standard 8VAC20-790-410-A-1
A first aid kit must contain at a minimum a digital thermometer. The digital thermometer was non-operable.
Plan of Correction: I will buy either a new battery or a new thermometer.
