Inspector Notes
A code compliance inspection was initiated and concluded on 4/10/2023. There were 6 children present, with 2 staff supervising. The inspector reviewed compliance in background checks and requirements for child day centers operated by religious institution exempt from licensure. A total of 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable code sections and violations were documented on the violation notice issued to the program.

Based on record review and interview with staff, the center did not ensure all applicants for employment, employees, applicants to serve as volunteers, and volunteers shall undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence: The Director confirmed that the fingerprint check (dated 3/5/2018) for Staff #1 was more than five years old.
