Inspector Notes

An unannounced inspection was initiated on 8/12/25 and completed on 9/17/25 with the local department of social service in response to a complaint received by the licensing office on 8/8/25 relating to behavioral guidance and forbidden actions. The inspector reviewed one child's record and one staff record on site on 8/12/25. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/24/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-210-A-4-a
Within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children, including but not limited to health and safety, child abuse and neglect, child development and playground safety for staff functioning as a lead teacher.

Staff A was hired as a lead teacher however the staff did not have the 24 hours of lead teacher training hours. This training was overdue by 17 months.
Plan of Correction: Once we became aware of the issue, we acted immediately. The staff member was placed on administrative leave during our internal investigation. In addition, we conducted a full review of all employee files to verify that every current staff member has completed all required training and certifications. The staff member is no longer employed at our location.

We take this matter seriously and have strengthened our internal systems to ensure oversights like this do not occur in the future. We have implemented checklists that outlines every required training for a teacher to be considered a lead. Before a teacher is promoted to a lead position, a member of management will go through the checklist and verify that teacher has completed all
necessary requirements. Our team remains committed to transparency, accountability, and continuous improvement. When management became aware of the concern the staff member was immediately placed on
administrative leave while we conducted an internal investigation. The employee is no longer employed by Everbrook Academy.
Standard 8VAC20-780-400-A
In order to promote the child's physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, support .

Child A, two years of age, fell after loosing their balance when the staff took an object from the child. The child fell to the floor and appeared upset and the staff provided no help or comfort to the child after the fall.
Plan of Correction: Management conducted one-on-one conversations with all staff members, as well as a group training session to reinforce our Positive Guidance and Positive Behavior Support policies and expectations, particularly regarding how we comfort, engage with, and support the children in our care on both August 8??, 2025 and August 14??, 2025. These steps were taken to ensure that every
staff member is clear on our standards and equipped to uphold them consistently. When management became aware of the concern the staff member was immediately placed on administrative leave while we conducted
an iinternal investigation. The employee is no longer employed by Everbrook Academy.
