During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

The inspection findings have been updated as a result of a second step review completed on 12/2/25.

An unannounced inspection was initiated on 8/25/25 and concluded on 9/23/25 with local agencies in response to a complaint received by the licensing office on 8/22/25 relating to allegations of a forbidden action, rough handling. The inspector reviewed one staff record on-site. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/30/25. A POC submitted after this date will not appear on the public website.

The following action or threat thereof is forbidden: rough handling of a child.

On 8/19/25 at approximately 3:55pm, Staff #1 roughly handled Child A. Observed evidence revealed that Child A was skipping past Staff #1 who was sitting in a chair. Staff #1 grabbed the upper arm of Child A and pulled the child over.
Plan of Correction: The child was observed skipping around the classroom, posing a safety risk to both themselves and others. The staff member reacted quickly out of concern for safety. That being said, corrective accountability measurements were also taken with the staff member. For the purpose of ensuring clear understanding of VDOE regulation and LCG policies and procedures, management completed a
retraining with staff on positive redirection, as well as taking the time to pause, assess, and respond thoughtfully rather than react instinctively in the moment. This training was completed on 09/10/2025.
