Inspector Notes

A monitoring inspection was initiated on February 21, 2023 and concluded on February 21, 2023. There were 26 children present, ranging in ages 6 months to 4 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard 8VAC20-780-240-A
Based on record review and staff interview the center did not ensure The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Evidence: The director confirmed that Staff #5 (hire date 6-29-2022) completed the orientation course January of 2023.
Plan of Correction: Staff member has completed training. The director will ensure future staff will be more forcefully required to complete the training.
Standard 8VAC20-780-40-K
Based on staff interview the center did not develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Evidence: The director confirmed they do not have a policy for shaken baby or abusive head trauma.
Plan of Correction: The owner will write a policy on shaken baby and abusive head trauma.
Standard 8VAC20-780-510-G
Based on medication review and staff interview the center did not ensure medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Two containers of prescription medication (Albuterol) was observed, and they were not labeled with the child?s name, dosage amount, or time to be given. The director confirmed both medications belonged to Child #6.
Plan of Correction: Medication sent home on 2/21/23. New medication with proper labeling received 2/23/23. The director will ensure future meds are labeled appropriately before being received.
