Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on July 6, 2022. The inspector arrived at 10:35 AM and departed at approximately 1:10pm. There were 26 children present and three staff members. A sample size of nine records were reviewed which included five children?s records and four staff records. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Based on record review and interview, it was determined that the center did not ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.
Evidence:
1. Two of the four staff members in which records were reviewed did not have the Virginia Preservice Training completed.
a. Staff #2 (date of hire: 07/21/21) did not have the preservice training and was present during the time of the inspection working with children.
b. Staff #3 (date of hire: 5/21/21) did not have the preservice training and was present during the time of the inspection working with children.
2. Staff #1 confirmed both Staff #2 and Staff #3 did not have their Virginia Preservice Training completed.
Plan of Correction: The center responded with the following: Staff #1 will have Staff #2 and Staff #3 complete the Virginia Preservice Training by the end of the week and obtain a copy of the certificate for their record.

Based on record review and interview, it was determined that the center did not ensure that within 90 days of the date of employment or 90 days from subsidy vendor approval that staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. Staff #2, (date of hire: 7/21/21) did not have a CPR certification.
2. Staff #1 confirmed Staff #2 did not have a current CPR certification.
Plan of Correction: The center responded with the following: Staff #2 is scheduled for her CPR training and certification on August 6, 2022. Once completed, Staff #2 will provide a copy for the center?s training record.
