Inspector Notes
An unannounced monitoring inspection was conducted on July 10, 2023. The inspector arrived at 12:50pm and departed at approximately 3:15pm. There were 27 children present and 5 staff members. A sample size of 10 records were reviewed which included 5 children?s records and 5 staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, free play, lunch, and nap. Handwashing was observed. First aid kits and emergency preparedness documents were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Evidence:
1. Four of the five staff records reviewed did not have results of the Virginia Central Registry in their records.
a. Staff #2 (date of hire 12/15/2022) did not have results of the Virginia Central Registry in their record.
b. Staff #3 (date of hire 10/22/2022) did not have results of the Virginia Central Registry in their record.
c. Staff #4 (date of hire 05/14/2021) did not have results of the Virginia Central Registry in their record.
d. Staff #6 (date of hire 04/17/2023) did not have results of the Virginia Central Registry in their record.
2. Staff #1 confirmed that there were no Central Registry results for Staff #2, Staff #3, Staff #4, and Staff #6 in their records.
Evidence:
1. Staff #5 (date of hire 7/10/2023) has a tuberculosis screening dated June 29, 2021 which was not completed within the last 30 calendar days of the date of employment.
2. Staff #1 confirmed that Staff #5 did not submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment.
Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. Staff #4 (date of hire 05/14/21) had an expired tuberculosis screening dated April 15, 2021.
2. Staff #1 confirmed that Staff #4?s tuberculosis screening was expired.
Based on observation and interview, it was determined that areas and equipment of the center shall be maintained in a safe and operable condition.
Evidence:
1. In the preschool room, the children?s table has three damaged area of deterioration that poses as an injury hazard to the children in care.
2. Staff #1, the owner, confirmed that the children?s table in the preschool room is damaged.
Evidence:
1. In 2 of the 4 classrooms at the center, there were no posted daily schedules in the two classroom.
2. Staff #1 and Staff #2 both confirmed that there were no daily schedules posted for two of the classrooms.
Evidence:
1. The changing pad in the children?s bathroom has tears to the waterproof lining making it an absorbent surface for diapering and changing.
2. Staff #2 confirmed the tears to the changing pad and that it is currently being used at the center.
Evidence:
1. Staff #4, lead teacher, did not have a current certification in CPR and the most current certification has an expiration date listed as June 2023.
2. Staff #4 is the only teacher in the classroom with no other staff members in the classroom that are CPR certified.
3. Staff #1 confirmed that Staff #4?s CPR certification is expired dated June 2023.
Evidence:
1. The emergency evacuation drill log has documentation of January 2023 and March 2023 drills. February 2023, April 2023, May 2023, and June 2023 were not documented to the emergency evacuation drill log.
2. Staff #2 confirmed that the drills from this year were not all documented.
