A subsidy health and safety inspection (SHSI) was conducted on April 12, 2024. The inspectors arrived at 11:15am and departed at approximately 2:15pm. There were 22 children present and 4 staff members. A sample size of 10 records were reviewed which included 5 children?s records and 5 staff records. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Based on observation and interview, it was determined that the center did not ensure that for general record keeping, each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. This shall be maintained and made accessible to department representatives.
Evidence:
1. For each group of children at the center, the teachers in each classroom confirmed that they did not have a written hard copy of record of daily attendance that documents the arrival and departure of each child in the care as it occurs.
2. Staff #1 and Staff #2 confirmed that there is no written copy for each group of children of a daily attendance that documents the arrival and departure of each child in care.

Standard 8VAC20-790-600-D-1
Based on record review and interview, it was determined that the center did not ensure that within 90 days of the date of employment that staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. Staff #4, (date of hire: 8/14/2023) did not have a CPR certification and was present at the center working with children.
2. Staff #1 confirmed Staff #4 did not have a current CPR certification and is working with children at the center.
