Inspector Notes

An unannounced, on-site renewal inspection was initiated on 08/19/2024 and completed on 08/19/2024, as a part of licensure period. The on-site inspection began at 9:42 AM and ended at 10:50 AM. The inspector reviewed compliance in the areas listed above. There were 15 children present (including 3 resident children) and 3 staff. The inspector reviewed 2 children?s records and 3 staff records, 1 resident record on-site. This inspection included document review,
interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation
Based on a review of documentation, the provider did not maintain a current Report of Tuberculosis Screening for each caregiver, every two years.
Evidence:
Caregiver A's TB result was available on file was dated 7/20/22 (expired).
Plan of Correction: I will have them make an appointment as soon as possible.
