During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/12/25, as a part of the licensure period. The on-site inspection began at 9:44 AM and ended at 11:18 AM. The inspector reviewed compliance in the areas listed above. There were 14 children present (including 2 resident children) and 3 caregivers; the point total was 37. The inspector reviewed 6 children?s records and 3 caregiver records on-site.
This inspection included; document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/25/25. A POC submitted after this date will not appear on the public website.
Violations
6Child #5's documentation of a physical was overdue by 11 months.
Two children did not have safety straps buckled while in a bucket seat at the feeding/activity table.
Child #6's record did not contain documentation of a medical insurance policy number.
Child #6's record did not contain written acknowledgment of the declaration of insurance.
Children # 1,2,6's records did not contain documentation of having viewed the verification of age and Identity within seven business days from the first date of attendance.
Child #3's updated immunizations were overdue by 18 months.
Child #4's updated immunizations were overdue by 11 months.
Child #5's updated immunizations were overdue by 3 months.
