During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 8/6/25 and completed on 8/12/25, as a part of the licensure period. The on-site inspection began at 11:50 AM and ended at 1:31 PM. The inspector reviewed compliance in the areas listed above. There were 15 children present, including three resident children and 3 caregivers. The point total was47. The inspector reviewed 12 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/19/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training,
caregivers shall obtain a minimum of sixteen clock hours of training annually in
areas relevant to their job responsibilities.
Caregiver A has completed only 13 of the 16 required annual training hours.
Plan of Correction: I am in process of completing the 3 hours course.
Standard 8VAC20-800-540-A
Repeat Violation
Feeding tables shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
Two children were observed seated in a feeding table, with no safety strap in use.
Plan of Correction: We will make sure to use seat belts when children use the seats.

The provider shall obtain the parent's written acknowledgement of the receipt of the declaration of insurance and provisions of the emergency preparedness plan.
Child #3's record did not contain acknowledgment of the declaration of insurance.
Child #7's record did not contain acknowledgment of the provisions of the emergency preparedness plan.
Plan of Correction: Both parents already brought me the signed form.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
Child #1's annual review was not available as the form was missing.
Child #9's annual review was overdue by 1 day.
Plan of Correction: The father has already updated and signed it.
Standard 8VAC20-800-80-A
Repeat Violation
Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the age and identity of the child.
*Child #1 had no documentation of verification of proof of birth.
*Child #2's verification of proof of birth did not document who viewed the information.
Plan of Correction: I'll have the father bring me a copy of the birth certificate.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed.
The emergency preparedness plan had not been reviewed annually. The document was dated 2023.
Plan of Correction: The 2025 emergency plan is already being updated.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain
documentation that the child has been adequately immunized.
Child #5's record had no immunizations documented.
Plan of Correction: The mother already gave me the updated vaccinations.

Repeat Violation
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Children #7,8 records did not contain documentation of immunization updates.
Plan of Correction: Announce to both parents that they are updating their vaccinations.
Standard 8VAC20-800-900-B
Children shall not be allowed to eat or drink while walking or lying down.
One child was observed walking around with his bottle.
Three children were observed lying down while drinking bottles.
Plan of Correction: We will be more careful and make the children drink their bottles while sitting at the tables.
