During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 8/6/25 and completed on 8/12/25, as a part of the licensure period. The on-site inspection began at 11:50 AM and ended at 1:31 PM. The inspector reviewed compliance in the areas listed above. There were 15 children present, including three resident children and 3 caregivers. The point total was47. The inspector reviewed 12 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/19/25. A POC submitted after this date will not appear on the public website.
Violations
10caregivers shall obtain a minimum of sixteen clock hours of training annually in
areas relevant to their job responsibilities.
Caregiver A has completed only 13 of the 16 required annual training hours.
Feeding tables shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
Two children were observed seated in a feeding table, with no safety strap in use.
Systemic Deficiency
A child's record shall contain the following information: parent's address, parent's workplace name and address, emergency contact information, authorization for emergency care and medical insurance name and number.

Child #9's annual review was overdue by 1 day.
Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the age and identity of the child.
*Child #1 had no documentation of verification of proof of birth.
*Child #2's verification of proof of birth did not document who viewed the information.
The emergency preparedness plan had not been reviewed annually. The document was dated 2023.
