Inspector Notes

A monitoring inspection was conducted on March 8, 2023 from 12:50 pm until 2:45 pm. There were fifty-four children present aged two to four years old, with five staff supervising. The
inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with appliable standards or law and violations were documented on the violation notice issued to the facility.

Based on the review of five staff records, the program failed to ensure that the results of a central registry check were completed within 30 days of employment.

Evidence:
1. The first day of employment for Staff #4 was 11/1/22. The result of the check was dated 3/12/23.

Based on the review of five child records, the program failed to ensure that each child have a physical examination within thirty days of first day of attendance.

Evidence:
1. The first day of attendance for Child #2 was 11/11/21. The date the physical examination was received was 4/4/22.
2. The first day of attendance for Child #5 was 1/16/23. There was no physical examination in the child's record.
