During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on October 9, 2024, as part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as a result of the last inspection related to building maintenance, forbidden actions, fingerprint background checks, and ratio. The on-site inspection began at 3:46pm and ended at 4:15pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
