Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #4 and staff #5, employed for over two months, did not have a completed national criminal background check.

The center must request a search of the central registry prior to the employee's first day of employment. Staff #4 and #5, who had been employed for over two months, did not have a central registry search requested.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #4, enrolled for a month, did not have documentation of an immunization record on file.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #4, who has been enrolled for one month, and child #5, who has been enrolled for over two years.

Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member or independent contractor who meets the requirements in subdivision 1 of this subsection.
1. Child #3, enrolled for a month and present at the time of the inspection, requires a rectal emergency medication and there are no staff at the center certified to administer the medication.

Prescription and nonprescription medication shall be given to a child only with written authorization from the parent. Child #3, enrolled for 10 months, and child #6 did not have parent authorization for the medication they require.

A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

The following staff records shall be kept for each staff person that includes documentation the staff completed orientation training. Staff #4, employed for over two months, did not have documentation of orientation in their file.
