Inspector Notes

An unannounced renewal inspection was conducted beginning on May 22, 2024 beginning at 9:35 a.m. and ending at 12:15 p.m. Staff files were reviewed from 1:30 p.m. to 3:15 p.m.

There were 12 children present, ranging in ages from 13 months to 3 years of age, under the supervision of 4 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Five children's records, eight staff records including staff training hours and three medications and authorizations were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation and interview with staff, the center failed to obtain repeat background checks every five years as required for all agents. Evidence: Agent #1's most recent sworn statement was dated 12/6/2017. Agent #1's most recent national fingerprint background checks were dated 2/8/2019. Agent #1's most recent central registry results were dated 12/15/2017.
