Inspector Notes
A monitoring inspection was January 18, 2024, from approximately 12:40 AM to 2:00 PM. There were four children present, ranging in age from toddler to school -age with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition. A total of four child records and one staff record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

5Evidence:
The records for child #2 (enrolled: 10/13/23) and child #4 (enrolled: 1/04/23) did not have provisions of the family day home?s emergency preparedness and response plan.
Evidence:
The record for staff #1 did not have documentation of a current Tuberculosis Screening.
Evidence:
In the bathroom children use, there were cleaning chemicals such as Lysol, Ajax, and Alcohol wipes located in a bottom cabinet accessible to children.

The last update, in writing was dated 09/01/22. The provider stated that the plan had not been reviewed.
