Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on February 18, 2022. The provider was contacted by telephone and a virtual inspection was conducted. In the care of the provider and two assistants were 10 children, ranging in age from 7 months through 4 years (27 Points). The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, training, care of children, physical environment, nutrition and emergencies. Two child records, the provider's record and the record of two assistants were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The most recent negative TB screening for Staff #1 was 02/10/2020.
2. The most recent negative TB screening for Staff #2 was 01/27/2020.
2, The provider confirmed that neither staff member had obtained an updated TB screening.
Evidence:
1. The record for Child #1 did not contain the name, address or phone number of a second person for the provider to call in case of an emergency and the parent of the child could not be reached. Date of enrollment was 05/24/2020.
2. The provider verified that the information was not in the child's file.
