Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 01/22/2024 from 10:45 a.m. to 11:35 a.m. There were 11 children present, ranging in ages from 14 months to four years old with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation and interview, the provider failed to ensure that each caregiver completed the department?s health a safety and update annually.
Evidence:
1. The date of Staff #2?s last health and safety update training was 01/13/2021. Staff #2 confirmed that the training had not been completed annually.
