Inspector Notes

This inspection was conducted by licensing staff using an alternate on-site and remote protocol, including telephone contacts, documents review, interviews, and on-site observation of the program.

A hybrid monitoring inspection was conducted on 01/14/2022 regarding a possible self-reported incident. During the on-site visit, there were 36 children present, ranging in ages from 4 months through five years, with 7 staff supervising. The inspector reviewed compliance in the areas of physical plant and staffing and supervision. Also on 01/14/2022, a virtual remote inspection was conducted were two children's records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with a non-incident related standard and the violation was documented on the violation notice issued to the program.

Based on the review of two children's records and verification from staff, the center failed to obtain an updated immunization record every six months for a child under two years of age.

Evidence:
1. The record for Child #1 did not contain one of the updated immunization records which are required every six months for children under the age of two. There was no updated immunization record from 06/18/2018 to 09/11/2019. Date of enrolled for Child #1 was 04/17/2018.
2. Staff #1 confirmed that there was one immunization record which had not been obtained.
Plan of Correction: This was an oversight. The staff understands that they need to get an updated immunization record which is signed by a physician or his designee every six months from the date of enrollment until a child turns two years of age.
