Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on March 16, 2022. There were 44 children present, ranging in age from 5 months to 5 years and in the care of 9 staff members. The inspector reviewed compliance in the areas of administration, physical plant, staff qualifications, staffing and supervision, programming, special services, medications, special care provisions and emergencies. A total of 3 children's records, 3 complete staff records, and 3 partial staff records were reviewed.

Information gathered during the inspection determined a non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the program.

Based on the review of three children's records and verification from staff, the center failed to obtain the immunization record for one child before the child attended the center.

Evidence:
1. The record for Child #2 contained a immunization record dated 12/09/2021. Date of enrollment for Child #2 was 11/15/2021.
2. The director confirmed that the immunization record was obtained late.
Plan of Correction: .In the future, the staff will ensure to obtain an immunization record showing the child is adequately immunized before a child's first date of attendance.
