Inspector Notes

An unannounced monitoring inspection was conducted on March 31, 2023, from 11:00 a.m. to 12:45 p.m. There were 35 children present, ranging in ages from six months to five years old with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and ten staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to ensure that criminal history background checks were repeated every five years.
Evidence:
1. Staff #4?s criminal history background check on file was dated 3/7/2018.
2. Staff #1 confirmed that there was not an updated criminal history background check for Staff #4.
Plan of Correction: Staff #4 central registry background check was completion on 3/20/23. We are still waiting for results.
