On February 2, 2026, Child #1 (age seven months) received second degree burns on the side of the left leg (from mid-thigh to the top of the ankle) while in care of the center. Interviews revealed the burn is believed to be a result of the hot water from the bottle warmer dripping off the bottle onto the child?s leg as staff noticed the child's pants leg was wet and did not change the child, nor further investigate. The water temperature in the bottle warmer measured as high as 211 degrees Fahrenheit.
Plan of Correction: Immediately following the 02/02/2026 incident, the family was notified, a follow-up meeting was held to review the incident and corrective actions, and the family returned the child to our care the following day. Staff and Directors then conducted a safety review, and the bottle warmer was removed from service; manufacturer instructions were reviewed and posted. New bottle warmers with enhanced safety features were subsequently purchased and placed into service. New procedures prohibit children in the bottle preparation area and prohibit staff from holding a child while preparing or retrieving bottles; bottles will be wiped completely dry upon removal from the warmer and temperature verified by two staff prior to serving. Staff were retrained to respond immediately to any signs of discomfort, wet clothing, or possible irritation. Effective immediately, all staff must be 18 years of age and high school graduates or GED equivalent. Infant staff retraining was completed 02/04/2026, procedures were added to the Infant Manual and onboarding process, full compliance was achieved 02/05/2026, and ongoing leadership monitoring will include a documented monthly safety check of all bottle warmers to ensure continued compliance.
