During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 05/15/2025. The on-site inspection began at 11:45 AM and ended at 3:00 PM. The inspector reviewed compliance in the areas listed above. There were 26 children present with 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

When a group of children receiving care includes children from different age brackets, the age of the youngest child in the group shall be used to determine the staff-to-children ratio that applies to that group. There shall be one staff member to 10 children from ages 36 months to five years.
During lunch time there were two staff members on duty with 26 children, ages three years through school age for 12 minutes, after which there was one staff member with the 26 children for the remaining 30 minutes of lunch time

Children were observed being sent into the building from the side grassy play area without communication between staff to transfer supervision.
There were two school aged children alone together in the library without supervision.

Results of a child protective services central registry search were not in the record provided for staff 3, employed for over 1 year, and who was on duty alone with children during the inspection.
