Background checks must be conducted every 5 years.

Staff 6 is missing subsequent criminal records check paperwork in file as well as a subsequent sworn disclosure statement.

An employee or volunteer of a licensed or registered child day program or of a family day home approved by a family day system must not be employed or provide volunteer service until the agency or home has the person's completed sworn statement or affirmation.

One staff , staff 2, is missing the sworn disclosure statement.

A subsequent TB screening must be done every two years.

One staff, staff 3, is missing a subsequent TB screening.

The center shall have a qualified program director or a qualified back-up program director who meets one of the director qualifications who shall regularly be on site.

At the time of the inspection the director was not present and none of the other staff were designated as the person in charge.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Staff 4 and 6 do not have any training in the file for the last year, staff 6 does not have training for the last two years.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center has a room on the second floor of the building used for individual tutoring by a staff member. This space is not currently covered by the license.

Medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.

An EpiPen was stored in a backpack, accessible to the children.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the information regarding place of employment, emergency contacts and physicians information.

One child, child 1, is missing the information regarding the physician, parents work address as well as emergency contacts.
