Inspector Notes
An unannounced on-site focused monitoring inspection was conducted on 09/05/2024. The on-site inspection began at 9:45 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above. There were 39 children and 9 staff present. The inspector reviewed 5 staff records. The inspection included document review, a tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children; the documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence: The record for Staff 2 contained a documented hire date of 09/04/2024; the tuberculosis screening was dated 11/01/2023.
