Inspector Notes

An unannounced inspection was initiated on 01/10/2025 and completed on 01/15/2025 in response to a complaint received by the licensing office on 01/10/2025 relating to administration. The inspector reviewed 1 child?s record and 2 staff records on-site on 01/15/2025. The inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis screening or testing is required every two years from the date of the last screening.

The tuberculosis screening for Staff 1 was overdue by 10 months.
Plan of Correction: Will ask staff to get an updated TB screening.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children?s serious and minor injuries in which entries are made the day of the occurrence.

Child 1 (6 years old) sustained an injury while under the supervision of the center on 12/31/2024. There was not a written record of the injury.
Plan of Correction: Will be sure to do all injury reports.
Standard 8VAC20-780-80-B-2
The center shall inform the department?s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Child 1 (6 years old) sustained an injury, while under the supervision of the center on 12/31/2024, that required outside medical attention. The center did not notify the department that the injury occurred.
Plan of Correction: Going forward, will be sure to report injuries that required medical attention.
